(January 19, 2017)—Encore Healthcare, LLC, announced the purchase of Nexus, a HIPAA-compliant care management platform from Nashville-based Jumpstart Foundry alumnus, Evermind, Inc., accelerating its leadership, expertise and innovation in post-acute respiratory population health management.

Acquisition of the pre-market Nexus Respiratory Management software and intellectual property represent a significant expansion of Encore’s technology stack, allowing it to own a state-of-the-art assessment and plan-of-care platform on which to deploy its multisite respiratory programs for compromised cardiopulmonary patients and health plan members.

“With Nexus Respiratory Management, providers will be able to seamlessly deliver any Encore program at various intensities of care, whether in the rehab center, skilled nursing facility, or patient home, and on any device—laptop, tablet or smartphone,” said Encore CEO Zach Gantt.

With its scalable architecture and proprietary care protocols, Nexus was designed to allow post-acute providers to expertly manage patients with moderate to severe cardiopulmonary disease. Nexus enables providers to partner with payers as they transition from fee-for-service to value-based care. “The industry has needed an evidence based, data driven, useable platform that delivers optimal outcomes for this patient population, and Nexus fulfills that need,” said Encore Executive Medical Advisor, Dr. Doug Kane.

The acquisition agreement was approved by the Boards of Directors of Encore and Evermind, and closed in late November. Under the agreement, Evermind will continue to sell its family caregiving sensors directly to consumers, and will provide sensors to the healthcare industry through Encore.
